This binding prevents the movement of tRNA peptidyl molecules, causing the inhibition of peptide chain elongation and termination of protein synthesis. Due to its high concentrations in the respiratory, genital, and urinary tracts, azithromycin is effective in treating bacterial infections specifically in these areas. Mid-infrared spectroscopy has shown that azithromycin binds to the 50S subunit of ribosomes and interferes with the translocation step, crucial for peptide chain synthesis. It is considered a functional analog to erythromycin but has a broader spectrum against gram-negative organisms and improved resistance profile.
